Arms Acres (www.armsacres.com) - Carmel, NY
This 129-bed residential alcoholism and substance abuse treatment center offers comprehensive services by a multidisciplinary treatment team of physicians, psychiatrists, nurses, certified alcoholism and substance abuse counselors, social workers, family specialists and activities specialists. Their approach encompasses the physical, mental and spiritual aspects of recovery with an emphasis on the needs of each individual patient. The philosophy and approach of Alcoholics Anonymous and Narcotics Anonymous are fully integrated into the program. Arms Acres offers a 25-bed medical detoxification unit. At the conclusion of detoxification treatment, patients may be transferred to the Arms Acres Men or Women's Rehabilitative Programs or directly referred for outpatient follow-up. Services include psychiatric evaluation, acupuncture, individual and group counseling, psychosocial evaluation, aftercare planning, family programming and support and 12 step meetings. Fitness and recreation programs are available.
Cost: The program costs between $300 and $350 a day/$8505.00 for the full 28 day program. Detoxification services are $350 per day. Treatment is covered by most major medical insurance companies. Medicaid is accepted. Medicare is not accepted.


Ashley (www.fathermartinsashley.com) - Havre de Grace, MD
The Ashley program is based on a traditional approach to addiction treatment, addressing the physical, emotional and spiritual needs of the patient. Physical health is improved through fitness, diet and stress reduction. Spiritual and emotional health is supported through spiritual lectures, group and individual counseling, retreats and services as well as the use of the Twelve Step philosophy with nightly meetings and weekly step studies. Services include primary, relapse and family treatment as well as continuing care. A gym is available but exercise is an optional, not formal part of their program.
Cost: $16,640 is the published rate for the 28 day residential program. Scholarships available based on need and individual circumstances. Costs are covered by many major health insurance plans on a full or partial basis. Medicare and Medicaid are not accepted.

Betty Ford Center (www.bettyfordcenter.org) - Rancho Mirage, CA
The Center was founded in 1982 by former First Lady Betty Ford and Ambassador Leonard Firestone to provide residential treatment services for those recovering from drug or alcohol addiction. All programs begin with an individualized assessment and are based on the 12 Step programs. Up to 100 patients live at the Center between 25 and 32 days to begin the process of recovery. A structured program includes daily lectures, group therapy, and counseling sessions. Physicians, nurses, psychologists, spiritual care counselors, activity therapists, registered dietitians, and other staff work together to create individualized treatment plans for each patient and to evaluate their progress. Specialized support groups are available on an individual basis. The facility has a gym, pool and weight training which are used daily as part of the program. Following discharge, patients receive a continuing care plan and participate in the Focused Continuing Care program, staying in close contact with the staff for a full year after leaving the Center.
Cost: Approximately one month inpatient services $14,700; Relapse extended care $25,000-27,000. Private insurance is accepted. Financial assistance is available after a $3,000 co-payment for applicants that qualify.


Caron Foundation (www.caron.org) - Wernersville, PA
The Caron treatment program varies in length according to individual needs and is based on the 12-Step model. The residential program offers complete chemical dependency treatment services. It begins with a thorough medical evaluation and detoxification. Treatment combines individual and group therapy, therapeutic activities, daily lectures and continuing care planning for each resident during their stay. Appropriate referral to an individual's next level of care is made following residential treatment. Caron staff routinely keep in touch with former patients and track their progress and the program provides workshops and seminars to help former patients throughout their recovery. In addition specific residential and outpatient programs for relapse treatment are available that help patients understand and cope with relapse issues and develop strategies to address patterns of relapse.
Cost: Inpatient program is $950 a day for days one through three and $570 per day for days four and up. They will work with health insurance providers. Medicare is not accepted. Scholarships are based on financial need and availability.


Cri-Help (www.cri-help.org) - Los Angeles, CA
Cri-Help's residential program is divided into clinical phases ranging from a period of restriction to a gradual reintegration into the community. The program provides multidisciplinary treatment that includes a structured residential environment, drug and alcohol education including daily attendance of Narcotics Anonymous and/or Alcoholic Anonymous meetings, daily study groups on the 12-step recovery principles, life enrichment and socialization programs, problem solving and coping skills training, ongoing individual and group counseling, recreation, family support groups, remedial education and vocational assistance, and nutritional counseling. Acupuncture and neurofeedback services (a form of biofeedback found to be effective in addiction treatment) are also offered at the facility. A 14-bed medically managed detoxification program is available for private pay or medically-insured patients. Outpatient treatment following residential treatment is offered in order to provide a continuum of care. A gym is available on the premises.
Cost: The residential program is $4200/month. Cri-Help treatment is contracted and accepted by most major managed care companies. An initial assessment will be done in order to determine whether an individual is eligible for various subsidized funding sources. Medicare and Medicaid are not accepted.

Day Top has 13 residential centers offering treatment for adolescents and adults. Programs emphasize peer support and feedback in a highly structured family-like environment known as a Therapeutic Community. Day Top offers a multidisciplinary holistic treatment program including individual and group counseling, family therapy, art feelings workshops, informational seminars, medical and mental health services, relapse prevention support, vocational or job training, and recreational activities. Exercise is available but not mandatory, with the larger centers having gyms and exercise facilities and the smaller ones having exercise rooms.
Cost: $70/day for adolescents plus medical expenses, $60/day for adults plus medical expenses. A sliding fee scale is available for both adolescents and adults. Insurance is accepted. Eligible patients can apply for public assistance to cover the cost of treatment.

Fairview Recovery Services (www.fairviewbehavioral.org)
Fairview University Medical Center, Minneapolis MN
Fairview's approach includes the treatment of chemical dependency as an illness; the use of a multidisciplinary staff to create individualized treatment programs; the use of group therapy and the 12-step programs as primary treatment methods; and the inclusion of the family or support network in education and therapy programs. The program is geared towards healing the whole person and helps clients to develop new patterns of living and thinking that support sobriety. The length of treatment varies with each client. All will undergo an initial assessment and evaluation procedure. Following the intake process, the client is assigned to a treatment group and receives an individualized treatment plan including group therapy, individual counseling, family program, spiritual care, cultural services and education through lectures and selected readings. Inpatient services is a five-day program focused on medically supervised detoxification and inpatient and outpatient evaluation and referral. The Lodging Plus residential program focuses on group and individual therapy and family therapy. There are no gym or exercise facilities on the premises. In order to sustain recovery an individualized Aftercare Plan is available which includes group therapy sessions with a primary treatment counselor as well as regular attendance at the appropriate 12-step program meetings. In addition there are special residential substance abuse treatment programs for women, seniors, dual diagnosis and the deaf and hard of hearing.
Cost: Inpatient services are $841.00 per day for the first 3-5 days, $387 per day for the next 20 days. Many health care insurance providers cover most costs. Medicare is not accepted. No scholarships are available.

Hazelden (www.hazelden.com) - W. Palm Beach FL, Center City MN, Plymouth MN
Hazelden provides both residential and outpatient treatment for adults and young people with substance abuse problems as well as programs for families affected by chemical dependency. They use the Minnesota Model treatment approach based on the Twelve Steps of Alcoholics Anonymous, and emphasize an individualized and holistic approach. Services include assessment and rehabilitation for adolescents and adults, aftercare and family services, renewal services such as recovery retreats and relapse prevention programs, residential extended care services that offer additional time for recovery after a primary treatment program is completed, and continuing care. Physical fitness is emphasized but not mandatory, with both facilities having a gym and the Minnesota location having a pool. Their web site includes bulletin boards and a chat room, a large selection of books, and videotapes on addiction, an online newsletter, daily meditations, twelve-step recovery information, and articles on drug and alcohol addiction.
Cost: Four-week inpatient program in Florida is $20,000 and in Minnesota is $18,999. The program is not covered by most health insurance. Medicare is not accepted. However, Hazelden has various pricing tiers and extends $4 million in patient aid each year so many people unable to pay the full price can attend. .

Marworth Alcohol and Chemical Dependency Treatment Center

Founded in 1915, the Geisinger Health System is a physician-led health care system that consists of a variety of services, including the Marworth Alcohol & Chemical Dependency Center. Marworth offers a variety of specialized inpatient and outpatient treatment programs. Individual treatment plans are designed for each patient within the framework of the 12-step program. Special emphasis is placed on including the patient’s family in the recovery program. Recreation therapy is part of the daily program, but there is no physical work out equipment.
Cost: $2750 per week for approximately a four week program. Health insurance and Medicare are not accepted. Full scholarships are offered.


Odyssey House - centers in Louisiana, Michigan, New Hampshire, New York, Texas, Utah, Australia and New Zealand

Odyssey House is a drug-free, residential therapeutic community. Services consist of group, individual and family therapy, medical services, recreation, social skills training, vocational training and educational services as well as treatment for specialized needs and aftercare and transitional living programs. Treatment programs are available for adults and adolescents. The program in Utah has a work out facility but participation is strictly voluntary.

Cost: Utah program - $1,350 a month. A substantial portion is covered by Medicaid and the program will work with the patient’s private health insurance. Medicare is not accepted. Some donated money is available for the indigent. Louisiana, Michigan, New Hampshire, New York, and Texas programs - Start at $846 a month. They accept Medicaid, Medicare and private health insurance as full payment. There is a sliding fee scale for patients who are paying for services. No scholarships are available.


Phoenix House (www.phoenixhouse.org) - centers in California, Florida, Massachusetts, New Hampshire, New York, Rhode Island, Texas and Vermont
Phoenix House includes both adolescent and adult treatment programs that are based on the traditional “Therapeutic Community” method of treatment. This emphasizes mutual peer self-help to support recovery in a structured environment. There are three phases of residential treatment. Phase 1 is a 30-day orientation period and includes a full assessment. Phase 2 is the primary treatment period, usually between 9-12 months, consisting of intensive treatment including therapy groups and individual counseling and weekly seminars on various topics such as stress management, healthy relationships, family issues, life skills, and relapse triggers. In Phase 3 residents prepare to "reenter" society. Phoenix House has a small exercise facility that is available on a voluntary basis.
Cost: All costs are covered by Medicaid, welfare, government agencies and the Department of Corrections.

Samaritan (www.samvill.org) - Briarwood, NY
Samaritan Village residential treatment services provide intensive, highly structured treatment environments using the therapeutic community model (the principle of one recovering person helping another) as their foundation. Samaritan offers a wide array of treatment services, including group and individual addiction-focused counseling, mental health services, employment skills training and basic education. Treatment is usually between 12-18 months but is ultimately individually-based. In addition, short-term residential treatment ranging from 30 days to 6 months is also available for adults requiring structured support for recovery but who basically have stable home environments and employment or vocational skills. Residential centers have small exercise facilities and when necessary rent space in local gyms for voluntary exercise programs.
Cost: Most patients are covered by Medicaid. Patients who can afford a fee will pay on a sliding scale that is worked out on an individual basis.

Sierra Tucson - (www.sierratucson.com) Tucson, AZ
Sierra Tucson is an accredited special psychiatric hospital and behavioral health center. The basis for treatment of alcohol and other chemical dependencies stems from the Sierra Model® that integrates philosophies and practices from the medical, psychological, and self-help communities. The 12-Step philosophy is a foundation of the program. The Sierra Model® addresses the mind, body, spirit and emotions and uses a combination of both traditional and innovative therapies. Patients undergo an individualized treatment program which may include disease and recovery education, relapse prevention, 12-Step meetings, adventure and equine-assisted therapies, and grief and spirituality sessions. The hospital has a gym available for use.
Cost: $895.00 per day for 26 days. Small discounts are available. Very few health insurance providers will cover services. Medicare is not accepted. No scholarships are available.

Silver Hill - (www.silverhillhospital.org) New Canaan, CT
Silver Hill is a psychiatric hospital with an integrated chemical dependency treatment program consisting of an inpatient detoxification and rehabilitation program. Based on a 12-step model, the program consists of a multidisciplinary assessment, crisis intervention, individual, group and family therapy, skills training, and a strong emphasis on education, family involvement and relapse prevention. Silver Hill has an indoor gym with Nautilus equipment, an indoor heated pool and sauna.
Cost: Approximately $1000 a day. Will work with private insurance and Medicare. No Medicaid and no private scholarships offered.

Smithers Center is the Division on Substance Abuse within the Department of Psychiatry at St. Luke’s/ Roosevelt Hospital in New York City. It has provided substance abuse treatment services for over 30 years. The program begins with a comprehensive assessment by a team of physicians, nurses, psychologists, social workers and substance abuse counselors. Acute inpatient care is available at the St. Luke’s site and sub-acute inpatient care is offered at the Roosevelt site. Detoxification/stabilization services are offered at both locations. Smithers inpatient rehabilitation services offer a safe supportive environment for discontinuing problematic alcohol and drug use through a variety of treatment options. These include motivational enhancement, core cognitive/behavioral treatment, relapse prevention, exercise and nutritional guidance, self-help, and both vocational and family counseling services. The treatment approach is based on an individualized clinical assessment, a collaborative working relationship with the client, and a cognitive-behavioral treatment model that addresses the whole person and a wide array of effective treatment strategies. Twelve step groups provide supplemental support to the formal clinical programs.
Cost: Inpatient rehabilitation is $500/day; detoxification is $1600/day. Accepts Medicaid, Medicare, and most forms of private insurance. A sliding fee scale for self-payment is also available.

Spencer Recovery (www.spencerrecovery.com)

Spencer Centers offer a variety of treatment programs. The inpatient program consists of the three phases of medical detoxification, primary rehabilitation, and family days. Two types of detoxification programs are available: standard medical detox and rapid detox. Length of stay for this program varies based upon individual need. Primary care is a 30-day inpatient program that encourages patients to recover physically, emotionally, and spiritually. In addition to individual, group, and family therapy, their approach includes exercise, meditation, and nutrition. The Centers offer a gym, personal trainer and yoga classes. Extended care that can last for up to another 60 days and long-term sober living facilities are also available to support patients who have completed primary care programs.
Cost: $12,000 and up for 28 day inpatient program. Treatment is covered by most insurance. Medicaid and Medicare is not accepted. No scholarships are available.

Turning Point (www.tpoftampa.com) - Tampa, FL
Turning Point of Tampa's inpatient program consists of three separate levels of residential treatment with special emphasis on a 12-Step approach to addiction as well as life skill education for successful recovery. Treatment is based on a holistic approach emphasizing the physical, emotional, medical, and spiritual needs of the whole person. Clients follow a treatment plan that is tailored to fit their own special needs and includes group and individual counseling. While exercise facilities are not available on the premises, patients can use outside gyms after the detoxification period. The level of care and length of stay are decided on an individual basis. In addition, chronic relapse prevention techniques are an essential part of the clients' treatment recovery program.
Cost: Inpatient services are $695 a day for approximately 38 days and are mostly covered by private health insurance. Fee for patients without health insurance is $8000 for 30 days. Medicare is not accepted. No scholarships are available.


Valley Hope (www.valleyhope.com) - Chandler AZ, Parker CO, Atchison, Halstead and Norton KS, Boonville MI, O’Neill NE, Cushing OK, Grapevine TX
Eight treatment centers are freestanding residential facilities; the Halstead facility is a freestanding chemical dependency rehabilitation unit at Halstead Hospital with programs for alcohol, cocaine and heroin addiction. Residential services include subacute detoxification, residential inpatient care, and continuing care treatment services. A medical, psychological and social assessment is completed at admission. A multidisciplinary team of certified addiction counselors, chaplains, physicians, nurses and psychologists work together to develop an individualized treatment program. This includes individual, family, couples, and pastoral counseling as well as group therapy settings totally approximately 47 hours per week of active treatment therapy. Participants attend educational and 12-step orientation lectures and local 12-step meetings. Some of the centers have exercise facilities available.
Cost: Fees are determined by the length of stay but in general up to 30 days is approximately $8,000. Public assistance coverage and Medicare are not accepted. Some private insurers cover total costs, others cover a portion. Aftercare services are free. A scholarship program is available with eligibility and amount determined at each residential facility. Approximately 200 scholarships are currently available each year.
